In addition, the club has prepared a form for trampers to record their own medical details and known medical conditions. This form is to be completed and kept in your pack, in a waterproof plastic bag. On the outside of your pack secure a plastic key tag with the label "MEDICAL DETAIL INSIDE PACK", on the zip to the compartment with the medial details. This form is printed by the club and provided to new members, or can be printed from this copy.
Note that this form is kept in your pack, and is generally not to be seen by others except when an emergncy occurs and you are unable to care for yourself. However if you have any medical conditions that you believe may be a risk to others, or yourself, then advise the Tramp Leader, who has to take reasonable account of the risks associated with the tramp.

In addition the club has prepared a form for use in an emergency, to assist in providing appropriate patient care and planning for safe rescue. It is intended that trampers carry two copies of this form in the plastic bag with their own medical details, so that in an emergency the form can be used to assist the Care Party leader.
